Ovarian Torsion

An emergency where an enlarged ovary twists on its ligament, cutting off blood supply.

Ovarian torsion is a gynecologic emergency in which the ovary (and sometimes the fallopian tube) rotates around the supporting ligaments, obstructing its blood supply. It causes sudden, severe pelvic pain and requires emergency surgery to untwist (detorsion) and preserve the ovary.

In fertility patients, ovarian enlargement from stimulation or OHSS increases torsion risk. Large ovarian cysts, dermoid cysts, and hydrosalpinx also predispose women to torsion.

Prompt diagnosis by ultrasound with Doppler flow and timely surgery can usually save the ovary. Delayed treatment leads to ischemia and the need for oophorectomy (ovary removal).
